Forms that do not require approval:
- Resident Leave of Absence
- Residents taking extended leave, including parental, caregiver, or medical leave, must first exhaust all vacation, sick, and other leave available to all residents under institutional policy. Any leave taken beyond that allotted time requires an extension of training for the additional time away from the program, regardless of attainment of MAVs or completion of required rotations.
- Resident Resignation, Termination, or Delayed Start Date
- Resident Repeating First Year of Training Notification
- Change in Director, Administrator, or Hospital Name
- Voluntary Permanent Reduction in Residency Positions
